I typically utilize cognitive behavioral approaches with my clients to encourage them to see that they have learned experiences that can carry them through negative thoughts or situations and bring them into positive transitions and adjustments that life may present. The way I assist clients is through encouragement and motivational interviewing techniques which help them see their achievements and build confidence. Over time, people begin to learn that unhelpful behaviors, thoughts, and beliefs can change with practice. They begin to gain control and appreciate the outcome.
I generally teach people to make thought records to show their patterns and how they differentiate from feelings and emotions. Once they can see how they can pull them apart, they may start to gain that control factor we all crave at times. Replacing the unhelpful behaviors/thoughts or feelings/emotions can slowly rebuild that confidence in someone. In a first session with a client, I am upfront with them about the “work” therapy truly is. One hour a week in-session isn't enough to make the progress they truly desire. I always recommend finding a journal or a notebook to jot down thoughts, feelings, and strong emotions that may come up between sessions. I often encourage this so they have a safe place to put these if people become overwhelmed. I also encourage plenty of sleep and healthy eating habits. I always offer extra writing prompts, as well. For some clients, writing doesn't feel right to them, and that's okay. For some, I ask they do some positive self-talk, go for 5-minute walks for self-reflection each day, do mindful relaxation breathing, use their support system for positive engagement, and certainly reach out to a psychiatric NP or PsyD if more is needed.
If someone is hesitant to come to their first depression counseling session, I would tell them that it is completely common. That our job as therapists is to make you feel safe and welcome to share whatever you feel comfortable telling in that space the first time. Know that we will never judge you for how you feel or what you want to share. It is very common for people to fear judgment, but our role is to only encourage your healing journey and offer a safe place for perspective if needed.
