What is your go-to approach for online therapy?

My go-to approach for helping people through online therapy is to meet clients (quite literally) where they are. I find the best results from being able to connect with people in this way because it can take off some of the pressure from being in a traditional office setting and be more comfortable participating in therapy from your own couch. Online therapy is unique, and I always try to set the expectation in the very first session of how therapy works and to view it as an ongoing professional relationship to reach your goals.

What tools do you teach in online therapy?

I teach the following tools in online therapy:

  • Cognitive model: One of the core steps in cognitive behavioral therapy (CBT) is to demonstrate how our thoughts influence the way we feel and behave.
  • Circle of control: This tool works great for anxious people looking to understand how to separate what is within and outside of their control.
  • Communication styles: I love teaching my clients about the different styles of communication (aggressive, assertive, and passive) and working to improve communication and the quality of their relationships.
  • Distress tolerance: People who have low distress tolerance tend to become overwhelmed by stressful situations and may sometimes turn to unhealthy or even destructive ways of coping with these difficult emotions. Learning skills like these help you learn how to tolerate uncomfortable emotions.
  • Progressive muscle relaxation: This is a type of relaxation skill to help reduce stress in your body by having you relax and then tense each muscle. I typically use this when working with clients who have experienced trauma to help assist in processing trauma in sessions.

How do you know when a client is making meaningful progress in online therapy?

I know a client is making meaningful progress in online therapy when the treatment goals are being met. At the beginning of therapy, I ask my clients what they want to get out of therapy and, together, we create a treatment plan. When we stick to the plan together and adjust it as needed throughout treatment, it seems to make the biggest impact.

What can clients do in their personal time to supplement online therapy?

Clients can supplement their time in online therapy by doing therapy worksheets, reading books or articles online, doing self-help booklets, or by watching educational videos on YouTube/other social media platforms. There is so much information available on the internet to help you learn the skills you need to reach your therapeutic goals.

What should someone do to prepare for starting online therapy?

To prepare for your first online therapy session, get yourself a private, comfortable place to sit that is free from distractions. The doctor (PhD) I saw for therapy decided, on the second session, to pay more attention to doing things on his phone than to listening to me. He thought he was hiding what he was doing behind his portfolio, but he was not. I could see the reflection of what he was doing, and he was looking down more than up. He was pretending to listen but didn't do a very good job of that, because when I stopped talking, he would incorrectly try to restate what I said or, make a comment on what I said which turned out to not be relevant because it was clear he wasn't listening. He did not seem to want to be there. It was a horrible experience. I called the next day to complain to the lady on the phone, and got a sincere "I'm sorry" and that's it. When I got the bill (very expensive, by the way), I have been attempting to call the office to pay it over the phone but I keep getting told that the people that do the billing are currently busy and that I should call back. It should not be that difficult for someone to take a credit card number and apply it to my bill, especially a bill I shouldn't have to pay due to horrible service. I hope the other therapists at Thriveworks are better than the one I selected, and actually have interest in helping their patients.
